OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm in that; a 6-foot ladder was not fully opened, with spreaders locked and all feet contacting a firm level support surface during use. (a) Pallet Service Corporation located at 525 Hwy 12 East, Knapp, WI; On or about March 14, 2016, a 6-foot fiberglass step-ladder was setup in the closed position while being used for working at elevation.
