(a) West trim saw line, operators station; A floor hole opening at the edge of the operator's platform where boards are conveyed between the unscrambler and saw infeed conveyor was not covered or guarded to prevent employees from stepping into it. (b) West trim saw line, southeast side of trim saw; A floor hole opening between the operator's platform and another platform leading to the scrap conveyor was not covered or guarded to prevent employees from stepping into it. (c) West trim saw line, east side of trim saw; A floor hole opening between the operator's platform and another platform leading to the notcher station was not covered or guarded to prevent employees from stepping into it.

29 CFR 1910.219(f)(3): Sprocket wheels and chains which were seven (7) feet or less above floors or platforms were not enclosed: (a) West trim saw line; On or about January 12, 2023, power transmission chains and sprockets for the even-ender rollers were not adequately guarded to prevent accessing in-going nip/mesh points. On December 27, 2022, the west trim saw operator received partial finger amputation injuries when their fingers were pulled into in-going nip points of the chain and sprocket drive system as they were adjusting the position of a board on the saw in-feed conveyor.
