29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents: (a) Brickmont Assisted Living, north exterior wall: On or about March 29, 2016, and at times prior, the employer failed to provide protective eyewear to employees operating pneumatic nail guns operating at 120 PSI. Employees were exposed to struck-by hazards which included plastic debris and fasteners as safety glasses or other eye protection was not provided.

(b) Brickmont Assisted Living, north side roof dormer: On or about March 29, 2016, and at times prior, the employer exposed employees to fall hazards of 21-feet, 6-inches working from a 6/12 pitched roof, in that personal fall arrests were not provided to employees while installing siding.
