29 CFR 1926.502(i)(3): All covers were not secured when installed so as to prevent accidental displacement by the wind, equipment, or employees:   At the Grady Square Apts., Section 7, located at 2511 North Grady Avenue,, Tampa, FL 33697.  (a) For employees that were working on the roof the HVAC hole was covered with plywood that was not secured to the floor, resulting in an employee from another company to fall 43'-7" through the HVAC hole, on or about 03/21/2016.

(a) For employees that were working on the roof, the HVAC hole was covered with plywood that was not marked with a  warning word, resulting in an employee from another company to fall 43'-7" through the HVAC hole, on or about 03/21/2016.

At the Grady Square Apts., Section 7, located at 2511 North Grady Avenue,, Tampa, FL 33697.  (a) For employees that were trained in fall protection no written (training) certificate record was provided with the signature of the trainer, observed on or about 03/30/2016.
