29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a. On the 5/12 pitch roof, fall protection was not provided to employees doing framing work, carrying plywood, and installing plywood sheeting, exposing the employees to a fall hazards between 19 feet 4 inches and 8 feet 8 inches, on or about 6/2/16.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a. At the jobsite, the portable extension ladder used to access the second story roof area extended approximately 20 inches or 1-1/2 rungs past the upper landing surface, exposing the employees to a 19 foot 4 inch fall hazard, on or about 6/2/16.
