29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.9 (c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent:    a)    The employer did not implement a Hearing Conservation program with all of its requirements (such as provision of suitable hearing protectors, medical monitoring, recordkeeping, and training).  An employee operating a Federal Mechanical Press (#750) without hearing protection was exposed to a cumulative noise dose of 293.9% of the permissible daily noise exposure or an equivalent sound level of approximately 97.8 dBA during a 523 minute sampling period on June 22nd, 2016.

29 CFR 1910.95(g)(6): At least annually after obtaining the baseline audiogram, the employer did not obtain a new audiogram for each employee exposed at or above an 8-hour time-weighted average of 85 decibels :  (a)   An employee operating a Federal Mechanical Press (#750) without hearing protection was exposed to a cumulative noise dose of 293.9% of the permissible daily noise exposure or an equivalent sound level of approximately 97.8 dBA during a 523 minute sampling period on June 22, 2016. The employee did not receive an audiogram since 2013.  (b)  An employee conducting metal finishing work was exposed to a cumulative noise dose of 60.99% of the permissible daily noise exposure or an equivalent sound level of approximately 86.4 dBA during a 524 minute sampling period on April 13, 2016. The employee did not receive an audiogram since 2013.  (c) An employee conducting metal flattening work (flattener) was exposed to a cumulative noise dose of 178.8% of the permissible daily noise exposure or an equivalent sound level of approximately 94.2 dBA during a 462 minute sampling period on June 22, 2016. The employee did not receive an audiogram since 2013.  (d) An employee conducting metal polishing work was exposed to a cumulative noise dose of 87.5% of the permissible daily noise exposure or an equivalent sound level of approximately 89.0 dBA during a 535 minute sampling period on June 22, 2016. The employee did not receive an audiogram since 2013.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    In the welding and polishing work areas, machinery guards were not in place to protect employee's hands and clothing from rotating machine parts and ingoing nip points:    (a ) The Wilton horizontal belt sander was missing a guard to protect employees from ingoing nip points at the rotating drums that drive the sanding/polishing belt.    (b) The Baldor vertical belt sander was missing a guard to protect employees from ingoing nip points at the rotating drums that drive the sanding belt.    (c) The buffing machine polishing wheels were missing guards to protect the threaded shaft ends and nut, exposing employees to a catching/caught-on  hazard.
