29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact.    A. J & M Builders LLC worksite located 8740 Treviso, Monclova, Ohio: On or about March 18, 2016, the employer did not ensure employees were wearing proper eye protection while operating pneumatic nail guns to secure sheets of plywood to a residential structure. The employees were exposed to potential flying object hazards while operating the pneumatic fastener.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.    A. J & M Builders LLC worksite located at 8740 Treviso, Monclova, Ohio: On or about March 18, 2016, the employer did not ensure employees were protected from falls while installing roof sheathing on the two story residential structure.  The employees were exposed to a fall distance of approximately 10.00 feet on the south side of the structure.

The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards.     A. J & M Builders LLC worksite located at 8740 Treviso, Monclova, Ohio: On or about March 18, 2016, the employer had not trained employees in the appropriate use of fall protection, such as but not limited to, the use of personal fall arrest systems while performing various framing operations.

29 CFR 1926.1053 (b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access.      A. J & M Builders LLC worksite located at 8740 Treviso, Monclova, Ohio: On or about March 18, 2016, the employer did not ensure the side rails of the aluminum ladder was extended at least three feet above the landing surface. Three employees were exposed to falls while using the ladder to access/egress the roof.
