29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  a.   On or about March 31, 2016, at the Barrett's Park development jobsite, four framing employees using pneumatic nailguns to install framing and roof sheathing were not protected from eye injury hazards such as flying debris or broken nails in that they were not wearing protective eyeglasses.

29 CFR 1926.1051(a): Stairways or ladders were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist was provided:  a.  On or about March 31, 2016, on the roof of the house at lot 91, employees accessing the 6:12 pitch roof to install OSB board roof sheathing climbed wall framing and bracing in that were not provided a ladder or stairway to access the roof eave, exposing them to an 8 feet fall hazard.

29 CFR 1926.501(b)(13): 29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, a personal fall arrest system, or an alternative fall protection measure as provided by another provision in paragraph (b) of this section:  a.   Employees installing framing and sheathing at elevations of six feet or more above the ground are not being protected from fall hazards by a fall protection system:  The employer fails to assure effective protection against falls as required by the standard, as demonstrated in the following instances on March 31, 2016 at a job site in the Barrett's Park development in Panama City, Florida:      1.  On or about March 31, 2016, on the 6:12 pitch roof at lot 91, 7160 Riverbrooke Street, two framing employees installing roof sheathing were exposed to an 8-12 feet fall hazard from the eaves and trusses in that they were not protected by a fall protection system.      2.  On or about March 31, 2016, on the second floor of the house at lot 90, 7158 Riverbrooke Street, two framing employees installing floor decking and framing were exposed to 10 feet fall hazards from unprotected edges of the second floor in that there were no guard rail installed around the open edges and stairwell and there were no other fall protection systems in use.  Halen Construction, Inc. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 988613, Serious citation number 01, item number 001 and was affirmed as a final order on October 24, 2014, with respect to a workplace located at Tidal Bridge Way, Lot 33, Watersound, Florida.

29 CFR 1926.503(a)(1): A training program was not provided to each employee who might be exposed to fall hazards, to enable that employee to recognize the hazards of falling, and be trained in the procedures to be followed to minimize these hazards:  a.  On or about March 31, 2016, at the jobsite in the Barrett's Park development, lot 90 and lot 91, employees performing floor decking, framing, and roof sheathing were not trained to properly inspect or recognize damage and wear to fall protection equipment in order to minimize fall hazards.
