29 CFR 1910.212(a)(3)(iii): Special hand tools for placing and removing material(s) was not provided to permit easy handling of material without the operator placing a hand in the danger zone: On or about March 15, 2016, the employer failed to ensure that the hand tool used on the hopper of the west grinding line was such that employee's hands were not in the danger zone. The open spacing on the grating of the interlocked door allowed for employee's fingers to be caught between the grating and the hand tool when positioning meat and plastic.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about March 21, 2016, the employer failed to ensure that machine guarding was in place to protect employees from the movement of the dump vat of the west grinding line. The movement of the dump mechanism created a pinch point between itself and the structural pieces of the hopper assembly.
