29 CFR 1926.1203(a): Before it began work at a worksite, the employer did not ensure that a competent person identified each space that was a permit space, through consideration and evaluation of the elements of that space, including testing as necessary:    (a)  Ambulatory Care Building:  On or about March 30, 2016 two maintenance employees were exposed to a burn hazard when the space they entered was not classified as a permit-required confined space by a competent person prior to the work being performed.  The employees entered into a Mechanical Air Intake Pit through an approximate three foot by two foot doorway at the bottom so that one employee could cut into and relocate both a steam vent line.
