29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. a) 150 Simpkins Rd: On or about August 18, 2022, an employee received an injury that required inpatient hospitalization after a same level fall. The section manager was aware of the hospitalization on August 18, 2022 but did not report to OSHA until August 22, 2022.

29 CFR 1910.29(f)(4): Openings in stair rail systems exceeded 19 inches (48 cm) at its least dimension: a) Main Office Area: On or about August 10, 2017 and times prior, employees used the stairway in the main office hallway to access managers offices and storage areas on the second floor. The openside of the stairway was not equipped with a rail system resulting in an opening of up to 3 feet between the side wall ceiling and the stair.
