29 CFR 1910.147(c)(4)(i): Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section. a. On or about March 28, 2016, the employer's written energy control program did not include step-by-step procedures to control and lockout all energy sources for employees who performed maintenance on the Pulp machine with Knife Gate valve. Employees were thereby exposed to the hazards associated with the unexpected energization of equipment.
