29 CFR 1910.23(c)(3): Regardless of height, open-sided floors, walkways, platforms, or runways above or adjacent to dangerous equipment, pickling or galvanizing tanks, degreasing units, and similar hazards were not guarded with a standard railing and toe board.    a. Production Area- On or about July 01, 2016, employees were required to work in and around an unguarded open pit filled with organic and inorganic sludge water that was four (4) feet deep and not guarded with a standard railing. Thereby exposing employees to fall hazard(s).    b. Basement of facility- On or about July 01, 2016, employees were required to work in and around an open pit that was four feet deep and not guarded with a standard railing. Thereby exposing employees to fall hazard(s).

29 CFR 1910.24(f): "Stair treads." All treads shall be reasonably slip-resistant and the nosings shall be of nonslip finish. Welded bar grating treads without nosings are acceptable providing the leading edge can be readily identified by personnel descending the stairway and provided the tread is serrated or is of definite nonslip design. Rise height and tread width shall be uniform throughout any flight of stairs including any foundation structure used as one or more treads of the stairs.    a. Stairwells throughout the facility- On or about July 01, 2016, employees were required to ascend and descend concrete stairwells daily. The tread on the concrete stairwells was badly worn or completely worn. Thereby exposing employees to slip and fall hazard(s).

29 CFR 1910.179(g)(2)(i): Electrical equipment was not so located or enclosed that live parts would not be exposed to accidental contact under normal operating conditions:    a. West-end of the Facility- On or about July 01, 2016, employees were required to use a custom built work platform 20Ton crane. The crane's pendant box had a selector switch button that was broken and being held together with black tape. Thereby exposing employees to shock and electrocution hazard(s).    b. East-end of Facility- On or about July 01, 2016, employees were required to use a Whiting 40Ton crane. The Allen Bradley pendant cable box on the crane had exposed conductors. Thereby exposing employees to shock and electrocution hazard.

29 CFR 1910.212(a)(1): Machine guarding.  Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.    a. Production Area- On or about July 01, 2016, employees were required to work near or around a wire machine on a daily basis. The wire machine had a series of unguarded in-running nip points. Thereby exposing employees to cuts, lacerations, and amputation hazard(s).
