29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents: a) Jobsite, Longvue Townhomes Lot 3 Cemetery Lane, Pittsburgh, PA - On or about April 1, 2016, an employee operated a circular saw to cut (pine) wood decking. This employee was not wearing eye protection such as safety glasses with side shields while exposed to flying debris.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure: a) Jobsite, Longvue Townhomes Lot 3 Cemetery Lane, Pittsburgh, PA - On or about April 1, 2016, employees were exposed to a fall hazard of up to approximately 19 feet to the ground below while engaged in framing activities on a sidewall of a residential townhouse.
