29 CFR 1926.405(a)(2)(ii)(J): Extension cords used with portable electric tools, appliances, or lighting, were not designed for hard for hard or extra-hard usage.     a) On or about 5 April 2016, an extension cord made of NM type cable was connected and energized by a generator to provide temporary power. Type NM cable is not designed for hard or extra-hard usage.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels were not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems.    a) Worksite - on or about April 5, 2016, employees were exposed to fall of 20 feet while clearing snow to prepare to install plywood roof sheeting. Employees did not use any fall protection.
