29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) Vertical Sample Machine - Honor Industrial Equipment: The in-feed feeder to the sample packaging machine did not have moving parts guarded adequately, exposing employees to a caught-in and pinch point hazard. (b) Advance Detection Metal Detector machine: Employees working on or around the infeed and out feed tail end of the conveyor, were exposed to ingoing nip points created by the conveyor belt and rollers. (c) Dura Pack Machine Operations: Employees working on or around the infeed and out feed tail end of the conveyors associated with the Dura Pack Machine process, were exposed to ingoing nip points created by the conveyor belts and rollers.

29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): (a) Vertical Sample Machine - Honor Industrial Equipment: On or about March 14, 2016, an employee operating/setting-up the vertical sample machine, was able to reach into the point of operation, exposing the employee to crushing and pinch point hazards created by the point of operation.
