An employee experienced a shoulder tear on March 30, 2016, and had days away from work, but was not recorded in column K on the OSHA 300 log presented on or about April 19, 2016.  (b) 2016 OSHA Form 300: The establishment incorrectly recorded a days away case. An employee experienced multiple contusions on March 30, 2016, and had days away from work, but was not recorded in column K on the OSHA 300 log presented on or about April 19, 2016.
