29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed: a) On or about April 4, 2016 one employee was observed by CSHO standing on a portable 8 foot A frame ladder, that he leaned against the rear wall of the structure in the closed position so he could mark out truss locations of the walls top plate. The employee was exposed to falls in excess of 5 feet to the surface below because the ladder was not being used as it was designed and/or intended.

29 CFR 1926.1053(b)(13): The top or top step of a stepladder was used as a step: a) On or about April 4, 2016, two employees working on the top plate of the exterior walls and did not have a ladder long enough to access the area where they were working. Employees used 6 foot A frame ladders to climb up onto the walls ledge exposing the employees to falls in excess of 6 feet to the concrete below.
