29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a) On or about April 5, 2016, two employees of GRC Construction, a Sub-Contractor for Raney Construction, Inc were observed by the CSHO installing roof sheathing and Truss framing while being exposed to a 9 foot fall hazard, because they were not protected by a fall protection system. There were no guardrails, safety nets, or personal fall protection equipment installed. The crew had personal fall protection in the work truck. b). On or about April 5, 2016, two employees of Orlando Framing Techs, Inc. a Sub-Contractor for Raney Construction, Inc were observed installing roof truss framing while being exposed to a 9 foot fall hazard, because they were not protected by a fall protection system. There were no guardrails, safety nets, or personal fall protection equipment installed.

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed: a) On or about April 6, 2016 one employee was observed standing on a portable 8 foot aluminum A frame ladder, that he leaned against the side wall of the structure in the closed position so he could hold a piece of material in place so the worker above him could nail it to the structure. The employee was exposed to falls to the surface below because the ladder was not being used as it was designed and/or intended.
