29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): Van's Lumber and Construction project site located at 5274 Bayshore Dr. Sturgeon Bay, WI: On April 7, 2016, employees were exposed to falls while building a two story steep pitch house in that: a) three employees were installing shingles without personal fall arrest systems or complete guardrail systems; b) an employee was working in the window opening on the second floor without any form of fall protection.
