Van's Lumber & Custom Builders, Inc. project site located at 3530 Mayapple Dr. Green Bay, WI 54311: On and before June 23, 2016, Van's Lumber & Custom Builders, LLC, as the Controlling Contractor, did not provide for frequent and regular inspections of it's jobsite to identify and address hazards that could cause injury and illness.
