C.B.S Boring and Machine Company, Inc. located at 2064 E. 2nd St. Defiance, OH 43512; On or about April 7, 2016, the employer did not ensure hazardous energy was controlled while installing a new fixture on the CNC G-machine. Employees were exposed to struck by hazards from the Fanuc Robot while installing the new fixture, in that the robot was not properly deenergized.  b.)    C.B.S Boring and Machine Company, Inc. located at 2064 E. 2nd St. Defiance, OH 43512; On or about April 7, 2016, the employer did not ensure hazardous energy was controlled while changing bolts on the main clamp of the CNC C-machine. Employees were exposed to struck by hazards from the Fanuc Robot while changing the bolts, in that the robot was not properly deenergized.  c.)    C.B.S Boring and Machine Company, Inc. located at 2064 E. 2nd St. Defiance, OH 43512; On or about April 7, 2016, the employer did not ensure hazardous energy was controlled while changing a prox switch on CNC C-Machine. Employees were exposed to caught by hazards from moving tools, in that the C-Machine was not properly deenergized.
