29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6 a.) C.B.S Boring and Machine Company, Inc. located at 2064 E. 2nd St. Defiance, OH 43512; On or about April 7, 2016, the employer did not ensure hazardous energy was controlled while installing a new fixture on the CNC G-machine. Employees were exposed to struck by hazards from the Fanuc Robot while installing the new fixture, in that the robot was not properly deenergized. b.) C.B.S Boring and Machine Company, Inc. located at 2064 E. 2nd St. Defiance, OH 43512; On or about April 7, 2016, the employer did not ensure hazardous energy was controlled while changing bolts on the main clamp of the CNC C-machine. Employees were exposed to struck by hazards from the Fanuc Robot while changing the bolts, in that the robot was not properly deenergized. c.) C.B.S Boring and Machine Company, Inc. located at 2064 E. 2nd St. Defiance, OH 43512; On or about April 7, 2016, the employer did not ensure hazardous energy was controlled while changing a prox switch on CNC C-Machine. Employees were exposed to caught by hazards from moving tools, in that the C-Machine was not properly deenergized.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a.) C.B.S Boring and Machine Company, Inc. located at 2064 E. 2nd St. Defiance, OH 43512; On or about April 7, 2016, the employer did not ensure employees were protected while entering the Robot Cell with the G-machine. One employee entered the cell to perform work on the G-Machine with the robot pendant, while another employee entered the cell behind him to watch the work being done, exposing him to struck by hazards from the Robot.
