29 CFR  1910.22(a)(2): The employer did not ensure that the  floor of each workroom is maintained in a clean and, to the extent feasible, in a dry condition. The employer did not ensure that when wet processes are used, drainage is be maintained and, to the extent feasible, dry standing places, such as false floors, platforms, and mats are provided.  a. C.B.S. Boring and Machine Company, Inc. - Defiance, Ohio: On or about December 3, 2025, the employer did not ensure that the work floor near the "Torque Box" conveyor for rejected parts ("WIP") and associated HMI station, was maintained in so far as possible a dry condition. Liquid which drips from the parts to the floor, and oil and wash water that is tracked out from the torque box cell, exposed employees to slip/fall hazards.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR  1910.22(a)(3): The employer did not ensure that walking-working surfaces are maintained free of hazards such as sharp or protruding objects, loose boards, corrosion, leaks, spills, snow, and ice.  a. C.B.S. Boring and Machine Company, Inc. - Defiance, Ohio: On or about December 4, 2025, employees who walked/worked on the conveyor roller lines such as, but not limited to, the LHTB line were exposed to injury from slips, trips, and falls.  The roller cylinders and gaps between the rollers created a hazardous walking/working surface which exposed employees to falling when accessing the conveyor line to align part racks.   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR  1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.  a. C.B.S. Boring and Machine Company, Inc. - Defiance, Ohio: On or about December 4, 2025, the employer did not ensure that powered conveyor roller lines such as, but not limited to, the LHTB line was guarded to protect employees from rotating parts.  Employees were exposed to fall hazards, pinch points, and being caught in the rotating parts when walking on the conveyor to align a part rack(s).   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
