29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to the joints or terminal screws: JOB SITE: 4350 Semple Avenue, Saint Louis, MO 63120, on or about 4/7/2016, a flexible cord located inside the newly constructed air compressor room, was being used to power an Air Heater, Model: MMHD1502TA, S/N: 016803, 1500w, 120v portable heater. The flexible power supply cord lacked strain relief on its female plug end.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket): JOB SITE: 4350 Semple Avenue, Saint Louis, MO 63120, on or about 4/7/2016, a blue flexible cord was not visually inspected before use for external defects and internal damage. The flexible cord was found being pinched between two doors leading to the compressor rooms 1 & 2 and the flexible power supply cord lacked strain relief on its female plug end.
