29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by rotating parts:  a) Worksite, Production Area/Converting Room: An employee was operating Pro Doing Center-Sealing Machine with exposed rotating parts where the material feeds into the center between the roll wheel and template. The employee was exposed to caught in and amputation hazards because the machine was not guarded to prevent employees from having any part of their body enter the danger zone during operation; on or about 4/8/16.  Note: In addition to abatement certification the employer is required to submit abatement documentation for this item in accordance with 29 CFR 1903.19.

29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):  a) Worksite, Production Area/Converting Room: The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The employer removed the guard, and thus did not ensure Ho Ting Micro-Computer Servo Control High Speed Cutting Machine (referred to as converting machine by employer) was guarded to prevent employees having any part of their body enter into the danger zone during operation; on or about 4/8/16.   Note:   Because abatement of this violation is already documented in the case file, the employer need not submit certification or documentation of abatement for this violation as normally required by CFR 1903.19.

29 CFR 1910.212(a)(3)(iii): Special hand tools for placing and removing material(s) shall be such as to permit easy handling of material without the operator placing a hand in the danger zone. Such tools shall not be in lieu of other guarding required by this section:  a) Worksite, Production Area/Converting Room: Employees were provided with hand tools for placing, removing, and adjusting materials from the point of operation. The hand tools were used in lieu of point of operation guarding. The employer did not ensure Ho Ting Micro-Computer Servo Control High Speed Cutting Machine (referred to as converting machine by employer) was guarded to prevent employees having any part of their body enter into the danger zone during operation; on or about 4/8/16.   Note:   Because abatement of this violation is already documented in the case file, the employer need not submit certification or documentation of abatement for this violation as normally required by CFR 1903.19.
