29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance shall be conducted at least once every three years.    a) Worksite, Buildings #20 and #24: Employees operating power industrial trucks such as but not limited to Yale Forklift (Model #ERP040VTN36TE084 - Serial #G807N03158J) and Crown Forklift (Model #SC4000) were not being evaluated; On or about 4/12/18.    NOTE: The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by moving parts:    1) Worksite, Production Area / Converting Room: Employees whose duties include operating DAH BAH Cutting Machine #3  were exposed to an unguarded moving part such as the horizontal perforation assembly; on or about 5/2/18.    Source One Packaging LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a) (i), which was contained in OSHA inspection number 1138748, citation number 1, item number 1(a) issued on 9/8/2016 and was affirmed as a final order on 10/4/2016, with respect to a workplace located at 20 Commerce Drive, Hauppauge, NY 11749.      NOTE: In addition to abatement certification, the employer is required to submit abatement documentation for this violation as required by 29 CFR 1903.19

29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):  a) Worksite, Production Area/Converting Room:  HOTING #2 Cutting Machine was not sufficiently guarded to prevent employees  from having any part of their body in danger zone during operation. There was about a two inches opening between the guard (hinge door) and the conveyor table; on or about 4/12/18.  b) Worksite, Production Area/Converting Room:  HOTING #2 Cutting Machine was not sufficiently guarded to prevent employees  from having any part of their body in danger zone during operation. Guard was not secured allowing employees to lift the guard during operation; on or about 4/12/18.  c) Worksite, Production Area/Converting Room:  DAB BAH Cutting Machine #3 was not sufficiently guarded to prevent employees  from having any part of their body in danger zone during operation. There was about 1 inch opening between the guard (hinge door) and the conveyor table; on or about 5/2/18.  d) Worksite, Production Area/Converting Room:  DAB BAH Cutting Machine #3 was not sufficiently guarded to prevent employees  from having any part of their body in danger zone during operation. Guard was not secured allowing employees to lift the guard during operation; on or about 5/2/18  e) Worksite, Production Area/Converting Room:  BJC - 500 PS Cutting Machine #5 was not sufficiently guarded to prevent employees from having any part of their body in danger zone during operation. There were approx. 2 to 3 in. opening between the guard (hinge door) and the conveyor table; on or about 4/12/18. There was approx. 1 in. opening between the guard (hinge door) and the conveyor table; on or about 5/2/18.  f) Worksite, Production Area/Converting Room:  BJC - 500 PS Cutting Machine #5 was not sufficiently guarded to prevent employees  from having any part of their body in danger zone during operation. Guard was not secured allowing employees to lift the guard during operation; on or about 4/12/18, 5/2/18.
