29 CFR 1926.451(c)(2)(v): The employer used forklifts to support scaffold platforms without ensuring that the entire platform was attached to the fork and that the forklift was not moved horizontally while the platform was occupied: On or about April 5, 2016, Elmer Miller dba Miller Building Systems, LLC does not require employees to secure the elevated supported scaffold to the rough terrain forklift by a chain or some other secure method. This violation was most recently observed when two employees were working from a supported scaffold at heights of approximately 10 feet on a residential construction site in Champaign, Illinois. This violation was most recently observed when employees were exposed to fall hazards of approximately 10 feet without fall protection on a residential construction site in Champaign, Illinois.
