29 CFR 1910.132(d)(2): The employer did not verify, through a written certification, the identity of the workplace evaluated, the person certifying that the evaluation had been performed, and the date the hazard assessment was done:  a)  At 1070 Technology Dr. North Venice (FL) on or about April 7, 2016,  the employer did not have an hazard assessment, such as but not limited to, in the New Vinyl WinGuard 1 Department, where employees were installing handles and sealant.  The PGT Industries, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.132(d)(2), which was contained in OSHA inspection number 936718, citation number 01, item number 2b and was affirmed as a final order on 12/20/13, with respect to a workplace located at 1070 Techology Dr. Venice, FL  34275.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  a) For New Vinyl WinGuard1 Department located at 1070 Technology Dr. North Venice (FL) where employees were using chemicals such as but not limited to Simsolv 50, Simsolv 252 and Dow Corning 791 Silicone Weatherproofing Sealant during window assembly, as observed on or about April 7, 2016.
