At the site, a two frame high tubular welded type scaffold had a 4 plank working platform exposing employees to a 13 feet fall hazard, on or about 04/07/2016.  b. At the site, a two frame high tubular welded type mobile scaffold had a 3 plank working platform exposing employees to a 13.5 feet fall hazard, on or about 04/07/2016.
