29 CFR 1910.146(g)(1): The employer did not provide training so that all employees whose work was regulated by 29 CFR 1910.146, Permit-required confined spaces, acquired the understanding, knowledge, and skills necessary for the safe performance of the duties assigned: (a) JBS Swift and Company @ 800 N. 8th Ave., Greeley, CO 80631: On or before January 20, 2016, as part of the injury/accident prevention responsibilities under its Confined Space Safety Program, the employer did not ensure that each employee involved in permit-required confined space entry had acquired the understanding, knowledge, and skills necessary for the safe entry. Training was inadequate in that employees were not provided with instruction on: 1) The accepted means of rescue and emergency services, including the need for mechanical devices to retrieve personnel from vertical type spaces more than 5 feet deep, and the correct means of connection to body harnesses (1910.146(k)(3). Condition exposed employees to confined space hazards.
