29 CFR 1904.7(b)(1): Employer did not ensure that a work-related injury or illness was recorded if it resulted in one or more of the following: death; days away from work; restricted work or transfer to another job; medical treatment beyond first aid; loss of consciousness; or significant injury or illness diagnosed by a physician or other licensed health care professional:  (a)  JBS Swift and Company, at 800 N 8th Avenue, Greeley, CO: On and preceding 5/23/16, the employer did not ensure that a work-related injury or illness was recorded if it resulted in one or more of the following: death; days away from work; restricted work or transfer to another job; medical treatment beyond first aid; loss of consciousness; or significant injury or illness diagnosed by a physician or other licensed health care professional.  Employees were treated for work-related burns at the JBS Swift and Company plant occupational health clinic.  The employees were treated with a mixture of bacitracin, burn gel, and Silvadene.  Silvadene is a prescription antimicrobial medication.  Administration of Silvadene was medical treatment beyond first aid.  The recordable work-related injuries were not entered on the employer's 2016 OSHA 300 Log.  (b)  JBS Swift and Company, at 800 N 8th Avenue, Greeley, CO: On or about 3/15/16, the employer did not ensure that a work-related injury or illness was recorded if it resulted in one or more of the following: death; days away from work; restricted work or transfer to another job; medical treatment beyond first aid; loss of consciousness; or significant injury or illness diagnosed by a physician or other licensed health care professional.  On 3/15/16 an employee received a work-related injury, laceration, that resulted in approximately two weeks of restricted duty.  The recordable work-related injury was not entered on the employer's 2016 OSHA 300 Log.
