29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: (a) 6th Floor: On or about April 08, 2016, and times prior, an employee cut into an overhead drain line causing stored Plug Buster drain cleaner, containing sodium hydroxide, to spray on his body. The employee was not wearing personal protective equipment such as, but not limited to, a chemical suit to prevent chemical burns.

29 CFR 1910.133(a)(1): Protective eye and face equipment was not required where there was a reasonable probability of injury that could be prevented by such equipment: (a) 6th Floor: On or about April 08, 2016, and times prior, an employee cut into an overhead drain line causing stored Plug Buster drain cleaner, containing sodium hydroxide, to spray onto the employees faces. The employees were not wearing proper face protection to prevent chemical burns.
