29 CFR 1903.19(c)(1): Within 10 calendar days after the abatement date, the employer must certify to OSHA (the Agency) that each cited violation has been abated, except as provided in paragraph (c)(2) of this section.   a. In regards to citation 1, item 1, issued on April 27, 2016, the employer has failed to submit abatement certification within ten days of the abatement date, June 1, 2016.  The violation read as follows:    29 CFR 1926.20(b)(1): It shall be the responsibility of the employer to initiate and maintain such programs as may be necessary to comply with this part:   a. On or before April 13, 2016, where employees conducted roofing work, the employer failed to initiate and maintain a fall protection program, thereby exposing the employees to fall hazards.  b. In regards to citation 1, item 2, issued on April 27, 2016, the employer has failed to submit abatement certification within ten days of the abatement date, May 17, 2016.  The violation read as follows:      29 CFR 1926.503(a)(1): The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards:     a. On or before April 13, 2016, where employees were working on the roof of a building, the employer failed to train the employees on the recognition of fall hazards and the procedures to be followed to minimize fall hazards, thereby exposing the employees to an eight foot, seven inch fall hazard.

29 CFR 1926.501(b)(10): "Roofing work on Low-slope roofs." Except as otherwise provided in paragraph (b) of this section, each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width (see Appendix A to subpart M of this part), the use of a safety monitoring system alone [i.e. without the warning line system] is permitted.  a. On or before April 13, 2016 where employees were roofing a church, the employees were not protected from falls, thereby being exposed to a 8 foot, 7 inch fall hazard.  John Smith was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 242966, citation number 1, item number 1 and was affirmed as a final order on June 2, 2012, with respect to a workplace located at 2808 Bryden Road, Columbus, OH 43209 .
