On or about April 11, 2016, at room 122 (storage room), employees were exposed to accumulation of contaminants such as mold (Hyphae/Conidiophores: Aspergillus, Penicillium, and Cladosporium-like spores) observed on medical equipment surfaces such as blood pressure cuffs and wheelchairs' fabric material.
