29 CFR 1910.22(b)(1): Where mechanical handling equipment was used, sufficient safe clearances were not allowed for aisles, at loading docks, through doorways and wherever turns or passage had to be made: On or about April 14, 2016, the employer did not ensure there was sufficient safe clearance through the doorway used to access the underside, pit area. Employees are required to lean under or wait for a gap between the heated parts of the moving overhead parts conveyor when passing through the doorway to access the underside pit area of the up-setter lines.

29 CFR 1910.23(a)(3): Every hatchway and chute floor opening was not guarded by one of the following: On or about April 14, 2016, the employer did not ensure there was a guardrail and/or cover over the chute openings. on the 53 up-setter line scrap and pull-out chute to protect the operator and other employees from or tripping on the chute opening when it is not in use. The wide of both chute floor openings measured approximately 23 inches wide by 16 inches long.
