NOTE: A sign reading DANGER -- PERMIT-REQUIRED CONFINED SPACE, DO NOT ENTER or using other similar language would satisfy the requirement for a sign.  On or about September 9, 2022, the employer failed to ensure that employees performing confined space entry first evaluated the space for hazardous atmosphere, nor did the employer ensure that employees continually monitored the confined space for hazardous atmosphere or other environmental hazards.
