29 CFR 1910.134(m)(2)(i)(B): The employer did not establish a record of the qualitative and quantitative fit tests administered to an employee which included the type of fit test performed.  a) On or about 4/19/2016, and at times prior to, at the U.S. Customs and Border Protection facility located at 236 Wendell H Ford Blvd in Erlanger, KY, employees were exposed to respiratory hazards (unknown chemicals and narcotics) by not conducting fit-tests on all respirators used by employees.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by rotating drill bit and chuck and/or flying chips.   a) On or about 4/19/2016, U.S. Customs and Border Protection employees were working at the facility located at 236 Wendell H Ford Blvd in Erlanger, KY, and were exposed to flying chips created by the un-guarded rotating bit of the 10 inch Dayton Drill Press.     Notice #1, Item #2, as required by 29 CFR 1910.212(a)(1): The employer failed to provide a guard for the moving chuck on a table top drill press.   The employer was cited for a violation of the same standard on 1/22/2016, Inspection # 1110421, date of final order, 2/18/2016, and means of abatement. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
