Kensington Dr., Appleton, WI 54915, Keller, Inc. employees were standing on and working from the top rails and intermediate rails of a SkyJack SJ 6826 RT Rough Terrain Scissor Lift which were each less than 18 inches wide.

29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems: On or about April 14, 2016 at a worksite located at 320 S. Kensington Dr., Appleton, WI 54915, an employee was not protected from falls of approximately 16 feet to the ground when engaged in aligning, affixing, and plywood sheeting steel roof trusses. The employee was provided with a personal fall arrest system including full-body harness and self-retracting lifeline; however, the employee was not connected to an anchorage point.
