29 CFR 1910.269(a)(2)(ii): Each qualified employee shall also be trained and competent in:  a. The skills and techniques necessary to distinguish exposed live parts from other parts of electric equipment.  b. The skills and techniques necessary to determine the nominal voltage of exposed live parts.  c. The minimum approach distance specified in this section corresponding to the voltages to which the qualified employee will be exposed, and  d. The proper use of the special precautionary techniques, personal protective equipment, insulating and shielding materials, and insulated tools for working on or near exposed energized parts of electric equipment:    On or about 11 April 2016, an employee was assigned to perform electrical servicing work on energized of the FF-005 without first having been provided adequate training pertaining to the respective job assignment. This exposed the employee to an arc blast/flash that resulted in burns to the face.     Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.269(d)(2)(iv): The procedure shall clearly and specifically outline the scope, purpose, responsibility, authorization, rules, and techniques to be applied to the control of hazardous energy, and the measures to enforce compliance including, but not limited to, the following:    a. A specific statement of the intended use of this procedure.  b. Specific procedural steps for shutting down, isolating, blocking, and securing machines or equipment to control hazardous energy.  c. Specific procedural steps for the placement, removal, and transfer of lockout devices or tagout devices and the responsibility for then, and  d. Specific requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures:    Wind Turbine Generator #E012- On or about 11 April 2016, an employee was exposed to energized electric parts of FF005 Turbine Converter cabinet without providing adequate procedures for the control of hazardous electrical energy "backfed" to the bus within the cabinet. Only network communication cables were required to be disconnected and the energy isolating device was not required to be used. This exposed employees to arc blast/flash hazards from backfeed through the bus.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.269(d)(6): The established procedures for the application of energy control (the lockout or tagout procedures) shall include the following elements and actions, and these procedures shall be performed in the following sequence:    (a) turn off and shut down the machines,  (b) physically locate and operate energy isolating devices in such a manner as to isolate the machine or equipment from the energy source,  (c) authorized employees affix lockout or tagout devices to each energy isolating device,  (d) all potentially hazardous stored or residual energy disconnected or otherwise rendered safe.  (e) verify that isolation and de-energization of the machine or equipment have been accomplished and test(s) performed to ensure that these parts were de-energized.    Wind Turbine Generator #E012 - On or about 11 April 2016, an employee was exposed to arc flash/blast hazards from energized parts within FF005 Turbine Converter cabinet while assigned to perform fuse removal, replacement, and other "final troubleshooting". Employees were not required to apply adequate energy controls prior to starting such tasks  and were exposed to arc flash/blast hazards from backfeed into the bus.     Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
