29 CFR 1910.134(f)(2): Employee(s) using a tight-fitting face piece respirator were not annually fit tested:  (a) VA Palo Alto, Building 100, MSICU: On or about March 18, 2016, the employer did not ensure that employees who were directly exposed to patients with active or suspected tuberculosis, were fit tests annually when required to wear tight-fitting negative pressure N95 respirators.                                                                                                                                            The U.S. Department of Veteran's Affairs was previously cited for a violation of this occupational safety and health standard, 29 CFR 1910.134(f)(2), which was contained in OSHA inspection number 517118, notice number 01, item number 001 and was affirmed as a final order on September 25, 2012, with respect to a workplace located at 2615 East Clinton Avenue, Fresno, CA 93703.
