29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about April 14, 2016, employees engaged in residential framing activities 6 feet or more above lower levels were not protected by guardrail system, safety net system, or personal fall arrest system.  Employees were exposed to the hazard of falls.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  On or about April 14, 2016, the employer did not provide a training program for each employee engaged in residential framing activities at heights greater than 6 feet.  Employees were exposed to the hazard of falls.

29 CFR 1926.404(b)(1)(iii)(C): The employer did not ensure that each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, were visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage.   A cord set, attachment cap, plug or receptacle of cord sets, or any equipment connected by cord and plug, that was found damaged or defective was used before it was repaired.  On or about April 14, 2014, the employer did not ensure that the following equipment was inspected before use, and equipment was being used before properly repairing the damage.    a)    Orange and black extension cord set b)    Cord on Makita circular saw  Employees were exposed to the hazard of electric shock.
