29 CFR 1926.28(a): The employer did not require the wearing of appropriate personal protective equipment in all operations where there is an exposure to hazardous conditions.    a) U.S. Coast Guard Navigation Center 7323 Telegraph Rd, Alexandria, VA 22306: On or about April 13, 2016, the foreman and other workers that applied Versico Roofing Systems Low VOC Bonding Adhesive which contained toluene and other hazardous chemicals, were not required to wear appropriate personal protective equipment, such as chemical resistant gloves, as required by the manufacturer and stated on the Safety Data Sheet in Section 7, exposing the workers to potential skin irritation, sensitization, and allergic reaction.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.  U.S. Coast Guard Navigation Center 7323 Telegraph Rd, Alexandria, VA 22306: On or about April 13, 2016, each employee that performed work on a low-slope roof with an approximately 16' parapet wall to ground fall distance was not protected from fall hazards by the use of guardrails, warning line system and safety monitoring system, or other means.  a) The employer did not have a complete guardrail system or other means of fall protection adjacent to the ladder point of access to the roof, on the approximately 66' wide middle roof section. Other areas on the middle roof section did not have complete guardrails or other means of fall protection. The parapet wall on the middle roof section was approximately 32" high.  b) The employer did not form an access path formed by two warning lines from the material handling area on the portico roof, where materials were lifted by a JCB telescopic handler, to the work area on the approximately 54'4" wide southwest roof section.  c) The employer did not form an access path from a material storage area on the approximately 54'4" wide southwest roof section where materials, such as but not limited to 5-gallon buckets, were stored to approximately 3 feet from the roof edge. Employees were not prevented by the configuration of the warning lines from accessing the materials or walking within 3 feet of the unprotected edge, exposing employees to a 16' parapet to ground fall distance.   Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement.

29 CFR 1926.1051(a): Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided:  U.S. Coast Guard Navigation Center 7323 Telegraph Rd, Alexandria, VA 22306: On or about April 13, 2016,  a) The employer provided a single extension ladder for access to the roof's parapet wall but did not provide suitable means to access the roof approximately 32" below. The foreman and other employees used a 5-gallon bucket, approximately 14" in height to step up and down from the approximately 10" wide parapet wall and the roof.  b) The employer did not provide suitable means of access to the southwest roof section from the middle roof section separated by a parapet wall approximately 32" high. The foreman and other employees used a 5-gallon bucket, approximately 14" in height to step up, exposing employees to slip and fall hazards.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement.
