29 CFR 1926.25(a): During the course of construction, alteration, or repairs, debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures:  (a)  Kensington Apartments:  On or about April 13, 2016, and at times prior thereto, employees were exposed to slips, trips and falls from the debris left in work area. 29 CFR 1926.1052(c)(1)(i): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with at least one handrail:    a) Apartment building T, stairwell:  On or about April 13, 2016 and at times prior thereto handrail systems were not in place on each stairway, exposing sub-contractors to fall hazards while using the permanently installed stairways for access to the second floor levels.     b) Apartment building O, stairwell:  On or about April 13, 2106 and at times prior thereto handrail systems were not in place on each stairway, exposing sub-contractors to fall hazards while using the permanently installed stairways for access to the second floor levels.     Note:  Abatement certification IS required for this item.

29 CFR 1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person:     a) Kensington Apartments:  On or about April 13, 2016, and at times prior thereto, the employer did not perform frequent and regular inspections of sites that they controlled and identify, correct and/or eliminate hazards to which their employees and/or the employees of subcontractors were exposed. Hazards include but are not limited to potential falls while using unguarded stairs, sub-contracted employees not utilizing safety glasses while working with pneumatic nail guns, slips and/or trips and/or falls due to poor housekeeping.     Note: Abatement certification and documentation are required for this item.    Bach Homes, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.20(b)(2), which was contained in OSHA inspection number 597278, citation number 1, item number 1a and was affirmed as a final order on 11/13/2012, with respect to a workplace located at 3400 E. River Valley, Meridian, Idaho 83646.
