29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents: On or about April 18, 2016, at the above addressed jobsite, employees were using and operating pneumatic nail guns and were not wearing safety glasses or other eye protection. Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: On or about April 18, 2016, at the above addressed jobsite, employees performing roofing work, twenty (20) feet six (6) inches, above the ground were not protected from falling. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
