29 CFR 1910.23(a)(9): Each floor hole, into which persons could not accidentally walk, was not protected by a securely held cover that leaves no openings more than 1 inch wide:  a) Employees were working on and from a dry dock deck with numerous unprotected floor holes in excess of 1 inch wide exposing the employees to tripping hazards.   b) Employees were traversing the dry dock area to access the dry dock deck via a steel ramp with numerous holes in excess of 1 inch wide exposing the employees to tripping hazards.     Condition occurred on or about 04/19/2016.
