On or about April 16, 2016, employees were exposed to fall hazards in the following instances: a. Employees were working on an open-sided BD mill II balance beam platform that was more than 4 feet higher than the work floor. b. Employees were working on top of the BD exit manipulators that the east, south, and west sides were more than 4 feet deep. c. The crane operator was working on a grating platform that had an approximately 18 inches wide open space to the southwest. The platform was more than 4 feet high.
