On or about December 20, 2016, at the shipping port area of the NYS in-plant rail operations, an employee Remote Control Operator ('RCO') wearing an OEM Cattron Theimeg remote control model MP96/48, Model Number 65540051 (the Cattron Remote Control Transmitter or "Cattron RCT") involved in the cited incident did not confirm that the Remote Control Locomotive 198 ("RCL, 198") had a functioning audible or visible alarm or warning device indicating to nearby personnel that RCL 198 was under active RCT and subject to movement before coupling/uncoupling of a railcar attached to RCL 198.
