29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:  a) Gym (T-75 Texas Ave Fort Campbell, KY) - On or about 4/13/16, employees were exposed to electrical hazards when the extension cord plugged into temporary power box (240/120 volts) was missing strain relief and was not tagged out.  ABATEMENT COMPLETED
