29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition.  On or about April 20, 2016, at the Imperial Beach Commissary Freezer Box 5 & 6, employees were exposed to slipping hazards from water freezing on the floor and ice chunks scattered on the floor.

29 CFR 1910.212(a)(3)(iii): Special hand tools for placing and removing material(s) was not provided to permit easy handling of material without the operator placing a hand in the danger zone:  On or about April 22, 2016, at the Imperial Beach Commissary Butcher Shop, employees were exposed to amputation hazards when making multiple cuts without using the provided pusher plate to move the meat across the saw.

29 CFR 1910.212(a)(3)(ii): The  point of operation of machines whose operation exposed an employee to injury, was not guarded:  On or about April 22, 2016, at the Imperial Beach Commissary Butcher Shop, employees were exposed to amputation hazards using band saws with the guard raised exposing approximately three inches of the blade above the material being cut.    The Defense Commissary Agency was previously cited for a repeat violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(3)(ii), which was contained in OSHA inspection number 1073789, citation number 1, item number 1, and was affirmed as a final order on August 11, 2015, with respect to a workplace located at 1300 Oceana Blvd., Virginia Beach, VA 23456.

29 CFR 1910.303(g)(1)(ii): The required working space about electric equipment rated 600 volts, nominal, or less to ground, was used for storage:  On or about April 20, 2016, at the Imperial Beach Commissary receiving warehouse, by the doors closest to the sales floor, shopping carts filled with merchandise were stored in front of electrical panels.    The Defense Commissary Agency was previously cited for a repeat violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(3)(ii), which was contained in OSHA inspection number 107019, citation number 2, item number 2, and was affirmed as a final order on February 17, 2012, with respect to a workplace located at 1500 13th Street, Bldg. 1264. Imperial Beach, CA 91932.
