Employer had not retained annual maintenance record for the fire extinguisher located on forklift. Fire extinguisher had mechanical damage to the bottom. Employees were exposed to struck-by and burn hazards

29 CFR 1910.157(f)(4): Portable fire extinguisher was not hydrostatically tested when it showed new evidence of corrosion or mechanical damage:   Inside Warehouse and Unloading 18 Wheeled Trucks: Yale Lift Truck Model ERC040VAN36TE082: Serial Number A969N01564J: DECA Property DE0094952: Employees were operating forklift to move pallets loaded with supplies. Employer had not conducted hydrostatic testing or removed from service a fire extinguisher located on forklift that had mechanical damage to the bottom of extinguisher. Employees were exposed to struck-by and burn hazards.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area.  DECA Meat Cutting Area: Employees used High Sierra Shimmer, Diversey J-512 Sanitizer, and Pure Bright Bleach to clean floors from spilled animal blood and to clean their cutting knives during and at the end of the shift.  The employer had not provided training specific to the health and safety hazards of these chemical. Employees were exposed to dermal, respiratory and eye hazards.
